Vision and Eye Insurance

At Modern Optometry, we will make sure that you’re getting everything possible out of your insurance coverage. If you’re not sure about your coverage, one of our opticians will be able to help you understand your benefits during your visit.

A few common questions:

How can I be sure I’m eligible for vision benefits?
If you have a vision card, please call the number on the back to speak to a representative. They’ll be able to confirm your benefits for you.

Am I limited to certain frames or lenses?
Typically, your insurance plan will contribute a certain dollar amount towards your frames, but other plans will restrict you to certain frames. We’ll be able to help you find a frame you love and will also work with your insurance company to make you happy!

What does my insurance plan cover exactly?
Refer to your vision coverage website and/or the phone number on the back of your vision insurance card for information regarding your benefits. We’ll also review them with you during your visit with us!

For us to confirm your eligibility (usually 24 hours prior to your appointment), we will need the following:

  1. Insurance ID Number of the member (can be found on your insurance card)
  2. Social Security Number of the member
  3. Birthdates of both the member and patient

Please remember to bring your insurance card to your appointment, and keep in mind that you’ll be expected to pay all copays, etc. at the time of your visit. Occasionally, an insurance company may pay less than expected. If this happens, you will be responsible for any difference.

Methods of Payment

We understand that the cost of vision care is important to you, and we’ll work with you and your insurance company to get you the most for your money. We accept cash, check, major credit cards including CareCredit. We can also discuss financing options with you during your visit.
