How do I know what is covered under my plan?

Most basic information can be found on your member website and at the 1-800 number on the back of your insurance card.

Can I choose any frame if I have insurance?

Most insurance plans allow you to choose your heart’s desire, but a few do limit your selection to certain brands. If you should find a frame that you simply cannot live without, we can usually find a way to make it happen.

How do I know when I am eligible?

The 1-800 number on the back of your card is your lifeline to members services.

What should I do with my insurance before I come in for my exam?

Call our office before your appointment to confirm if a claim form is necessary, and always bring your current insurance card with you.

Routine Vision Coverage

Some individuals are fortunate enough to have “Routine Vision Insurance” through their employer’s medical coverage policy. Some plans pay toward the optometric eye exam only, others will sometimes pay towards the contact fitting fee and some even pay toward materials. If you are uncertain about your benefit eligibility, you will need to check with your human resource department or call your insurance company directly and inquire about eligibility.

If your medical policy allows you to see any eye doctor of your choice we will verify your coverage, file the claim electronically and accept assignment of benefits for you.

To verify your vision coverage, we will need a phone number for verification of benefits, a group, plan or policy number, the social security number of the insured, member and patient’s date of birth, and name of employer. Verification of insurance benefits must be verified 24 hours prior to your appointment.

When you arrive for your appointment we will need a copy of your insurance plan policy card. You will be expected to pay in full the difference in the amount allowed at time of service. If for any reason the insurance company pays less than quoted you will be responsible for any amount unpaid.

Flexible Spending Accounts

Many employers are offering Flexible Spending Account (FSA) options to employees. Sometimes, these are referred to as cafeteria plans and are elective, supplemental insurance savings plans. These plans are designed to let you save money in an account, pre-tax, to pay for additional medical expenses such as eye exams, glasses, contacts, and often laser vision surgery. Check with the benefits administrator at your work to see if you are eligible for this program. FSAs can typically be used in conjunction with any vision insurance plan to offset any out-of-pocket costs on your behalf.

Other Payment Options

Our staff understands that cost is a concern when evaluating vision care providers. We do not want cost to be an obstacle to maintain proper eye health and good vision.
