Before you schedule, please note the following:
- Please check here to verify that we accept your insurance plan.
- Please note that we no longer accept Keystone First, Keystone Mercy, or Health Partners insurance plans.
- We accept patients 5 years of age or older.
- To reduce delays and/or avoid having to reschedule your appointment, please complete the patient intake form for each patient immediately after scheduling!
- If you schedule an appointment and fail to show or call to let us know you can’t make it, you will be charged a $50 fee per appointment.
Lastly, please answer the following question to the best of your ability, as there are different tests that are available, based on your needs.
Are you currently being treated for, or suspected to have, Macular Degeneration or Glaucoma and/or currently taking Plaquenil, Tamoxifen or Amiodarone?
